Surgery Refit - Moving back to Castle Street

We are nearly ready to move back to Castle Street Medical Centre after all the renovation work. The doors of your newly refurbished medical centre will be open again for patients on Monday 24 December from 08:00am, or 07:30am for those patients who have booked into extended hours. All your appointments from this time onwards will be back at Castle Street.

All your appointments up to and including Friday 21 December will be at Bolsover Hospital or Welbeck Road.

What is a Fair Processing notice?

A Fair Processing Notice is a statement by the Trust to patients, service users, visitors, carers, the public and staff that describes how we collect, use, retain and disclose personal information which we hold. It is sometimes also referred to as a Privacy Notice, Privacy Statement, Fair Processing Statement or Privacy Policy. This notice is part of our commitment to ensure that we process your personal information/data fairly and lawfully.

Medicines Management

The Practice may conduct Medicines Management Reviews of medications prescribed to its patients. This service performs a review of prescribed medications to ensure patients receive the most appropriate, up to date and cost effective treatments. This service is provided to practices within North Derbyshire and Hardwick Commissioning Group.

Medicines Order Line (MOL).

The medicines ordering service will issue medication and appliances on behalf of the practice whilst adhering to data protection and information governance. This service is provided to practices within North Derbyshire and Hardwick Commissioning Group.
